Not too worry you or anything, but I woud have him see an ear, nose, and throat specialist (otolaryngologist) and have them run a scope. A chest x-ray does not show the more serious things, although coughing can cause hemoptysis due to burst blood vesseld but, because he is a smoker, I would investigate further just be on the safe side. Sometimes ER does not order further tests and just do the quick tests and give medication and later on a person ends up in the hospital and then needs to see a specialist. I have seen in happen many times including myself. I went to the ER and told me I had nothing and I asked them to do more testing and they didn't and sent me home. Well, I was right. I ended up having to go back to the hospital days later and ended up having surgery. So when in doubt, it is better to be safe than sorry.
