Just a shot in the dark here .. Is it safe to assume you've tried doing a full body swaddle? Other than that, the only suggestion I have is about the pacifier. My daughter went through a mild phase where she would wake up when the binky fell out of her mouth. We started waiting until she was solidly asleep and then actually TAKING the binky out of her mouth. This way the sensation of it falling out won't wake her.
