sleep issues - poor self soother

Hello - my son is 12 weeks today. He is a very poor self soother and has a lot of trouble putting himself to sleep. He will even cry in the car if he is very tired until he falls asleep. For naps and night he will fall asleep in my arms or a swing. My problem is that for the past few weeks he has been waking up several times a night to fuss - not to eat. He only eats once a night ( which should be a godsend!)but he wakes about 10 - 15 times...every 45 mins to an hour and starts to cry until I move him into a new position ( bassinette to next to me, next to me to on my chest, chest to other side next to me) or he sometimes wants to be walked for a min and he passes right out. If I offer him a bottle he won't take it or falls asleep right away drinking. It gets really bad around 4:30 am and at that point I have been putting him in a swing I have next to my bed where he sleeps soundly until around 7:30 or 8:00 am.(swing is like a cradle - no chance of him falling out)

Has anyone else experienced this? I am baffled - I could understand if he wanted to get up and play oreat or even just be in bed with me but it feels like he can't put himself back to sleep without my help at night...any experiences or advice would help! Thanks!
